All of the following items must be received by Hope Technology School for your application to be considered complete.  When your application is complete, you may be contacted in order to schedule an informal interview (depending on classroom openings and availablility).  After this interview, your child's name will be moved into our application pool.  All applications for admission will be kept on file for two years, after which, re-application is necessary to remain in the applicant pool. 

To participate in our after-school Assistive Technology Lab, please submit only the AT Intake Form.

The admission process is as follows:

1. Fill out HTS application
2. If an openning if available, program manager will call to schedule a 30 minute informal interview with you and your child.
3. Student shadow for a day in the classroom
4. Decision by Admissions Committee
5. An official letter of acceptance will be given from HTS if this is an appropriate placement for your child

Submit items 1-7 below to complete your child's application:

 2. Latest report card and/or progress reports, including special services, and copy of current IEP, if applicable

3. Consent for Release of Information (click to download)

4. Consent and Release (click to download)

5. Child's Birth Certificate

6. Proof of address (a driver's license, utility bill with your name or copy of lease)

7. Current photograph of your child

After all of the above are complete:

 q       Intake interview and academic screening with Hope Technology School Staff.

 q      Due upon acceptance -  Current physical exam form from family physician or pediatrician including immunizations (click to download Medical Form, Immunization Form, and Immunization Schedule)

No application is considered complete without all of the above mentioned supporting documents.  Hope Technology School reserves the right to accept students based upon the admission committee's decision that the school is an appropriate educational placement for the child.

6. An official letter of acceptance will be given from HTS if this is an appropriate placement for your child

No student is officially accepted unless they receive acceptance in writing via a letter in the mail.
